Regarding your appointment:
- Please be sure to fax or mail authorization or referral forms (when applicable) to our office at least one week prior to the appointment.
- If you need to reschedule or cancel your appointment, we require 48 hours’ notice. Failure to notify us in a timely manner will result in no future appointments.
- If you are more than 25 minutes late, the appointment will need to be rescheduled.
For in-center speech-language evaluations:
- An extensive speech and language test will be completed, which takes around 2 hours. Observation rooms are available in our Charlotte office to watch the testing.
- Test results and recommendations will be discussed with you at the end of the evaluation. You will also receive a formal report in the mail in approximately ten to fourteen business days.
- Because of the extensive time involved with evaluation testing, if you fail to show for your scheduled appointment we will not reschedule.
For virtual speech-language evaluations:
- An extensive speech and language test will be completed, which takes between 1-2 hours.
- The parent or guardian will need to be present during the entire virtual evaluation.
- Following the evaluation, our Speech-Language Pathologist will discuss the results and recommendations with you. You will also receive a formal report in the mail in approximately ten to fourteen business days.
For school evaluations:
- An extensive speech and language test will be completed, which takes between 1-2 hours.
- The evaluation will be completed at the school.
- Following the evaluation, our Speech-Language Pathologist will call to discuss the results and recommendations with you. You will also receive a formal report in the mail in approximately ten to fourteen business days.

Charlotte Speech & Hearing Center is required by law to protect the privacy of your personal health information, provide this notice about information practices and follow the information practices that are described herein.
Uses and Disclosures of Health Information
Charlotte Speech & Hearing Center uses your personal health information primarily for treatment, obtaining payment for treatment, and evaluating the quality of care we provide. For example, we may use your personal health information to contact you to provide appointment reminders, or information about treatment alternatives or other health benefits that could be of interest to you. We may also use or disclose your personal health information without prior authorization for public health and auditing purposes, research studies, or for emergencies. We also provide information required by law.
Your Individual Rights
You have the right to review or obtain a copy of your personal health information at any time. You have the right to request that we correct any inaccurate or incomplete information in your records. You also have the right to request a list of instances where we have disclosed your personal health information for reasons other than treatment, payment, or other related administrative purposes.
You may also request in writing that we not use or disclose your personal health information for treatment, payment, and administrative purposes except when specifically authorized by you, when required by law or in emergency circumstances. We will consider all such requests on a case-by-case basis, but the Center is not legally required to accept them.
Concerns and Complaints
If you are concerned that Charlotte Speech & Hearing Center may have violated your privacy rights or if you disagree with any decisions we have made regarding access or disclosure of your personal health information, please contact our Executive Director at the address below. You may file a written complaint to the US Department of Health and Human Services. We will not retaliate against you for filing a complaint.
